       Our 183  bed  facility is  licensed by the State of New York and operates in compliance with New York State and Federal laws. We are dedicated to provide our residents with the optimum level of care for more than 53 years. Our staff members are highly trained, caring professionals who have many years of experience in the health care field. The nursing staff are regularly in-serviced on all aspects of care, using state-of-the-art nursing care, and as such are properly trained in providing our residents in a comfortable and dignified environment.

     We offer both private and semi-private accommodations. We accept Medicare, Medicaid, many private insurances as well as private payment. We will also work with your insurance to provide the highest level of quality care. Our Financial staff will assist with Medicaid Application as needed.

NURSING SERVICES

REHABILITATIVE SERVICES

Our on-site active program includes short and long-term physical, occupational and speech therapies adapted to facilitate the individual needs of each resident. Our goal is to enable our residents to achieve the greatest level of independence.

RECREATIONAL ACTIVITY

Daily programs include activities that meet the creative, emotional, intellectual, physical, social and spiritual needs of our residents. Special events and community outings are an integral part of our comprehensive program and an active volunteer program also complements our recreational activities.

DIETARY

Wholesome and nutritious meals are prescribed for regular and special therapeutic diets. Nutritional counseling is available from our registered dieticians who also supervise menu planning. Kosher dietary laws are observed.

SOCIAL WELFARE SERVICES

Services include counseling, problem solving and advocacy to achieve and maintain the highest level of physical, mental and psychosocial well-being of each resident.

OTHER BASIC SERVICES

Use of customarily stocked equipment, such as crutches, walkers, wheelchairs or other supportive equipment, including training in their use when needed. Activities program, including a planned schedule of recreational, motivational, social and other activities together with the necessary materials and supplies to provide a more meaningful quality of life.

Southern State Pkwy. to exit 19 South-Peninsula Blvd. Follow to end to Rockaway Tpke. To next traffic light and turn left onto Route 878 South (Nassau Expwy) follow directions above.

 

By Subway:

"A" train to Mott Ave. station (Far Rockaway). walk 3 block south to New Haven Ave.

By Train:

LIRR to Far Rockaway station. walk to Central Ave. And walk 4 blocks to New Haven Ave.

By Bus:

Queens: Jamaica bus line Q112 or Q113 to Mott Ave. station. Brooklyn: Green bus line Q35 to 116th street in RockawayPark. Change to Q22 to Mott Ave.

By car from Manhattan, Brooklyn or Queens:

Take Belt Pkwy. to Rockaway Blvd. South exit. Follow to Route 878 South 

(Nassau Expwy.) Make right at Central Ave. exit. Continue into Far Rockaway, where Central Ave. turns into Beach 20th St. follow to New Haven Ave. and turn right, go past stop sight to 22-41 New Haven Ave. on left side of street. parking is available in the rear lot.

A D M I S S I O N   P R O C E D U R E

To be considered for admission to Caring Family Nursing & Rehabilitation Center, a Patient Review Instrument (PRI) and Screen must be completed by a hospital or other transferring facility. Local community residents should contact the neighborhood visiting nurse services, or call us for further information. Upon receipt of the PRI and Screen forms, the applicant will be evaluated for medical approval, and may then be admitted to our facility.

Caring Family is a family-oriented Home Away from Home providing high quality care for over 53 years. We are constantly reinventing ourselves in order to meet the ever-changing need of our residents. We have recently remodeled our building to improve the comfort of our residents and to provide a home like environment. Since opening our doors, we have expanded our services to include Physical, Occupational, and Speech Therapy. We also offer Comfort/Palliative and Hospice Care Services.
